EFFECT OF ALLIUM CEPA (ONION) ON THERAPY OUTCOMES IN HEMIPLEGIC SHOULDER PAIN: A RANDOMIZED CONTROL TRIAL

Abstract


Abstract: Hemiplegic shoulder pain is the most common clinical consequences of stroke. It is a serious problem for both the patient and the health care providers as it reduces participation in rehabilitation, discourages movement, reduce activity of daily living, hinders recovery, adversely affects function and delay in discharge. Allium Cepa is a form of Aromatherapy that has antibiotic, anti-inflammatory and pain killing properties that have not been largely tested particularly in stroke survivors. Objective: To determine the effect of Allium Cepa on pain, upper-extremity function and quality of life in patients with HSP. Methods: Thirty participants were recruited in this randomised control trial. Participants were randomised into two groups, Topical Application of Allium Cepa Extract (TAACE) (15 participants) and Standard Physiotherapy Group (SPG) (15 participants). Measurements was performed at baseline, four weeks post intervention and at 12 weeks follow-up. Primary outcome measure is pain (visual analogue scale) and upper-extremity function (Fugl-Meyer scale). Secondary measure is quality of life (Stroke Impact Scale). Data was analyzed using descriptive and inferential statistics. Probability was 0.05. Results: There was significant improvement in pain level among the participants in TAACE group F1,14=420, p= 0.0001, but without improvement in the SPG group F1,13=0.62, p= 0.052. There was significant improvement in upper extremity function in the TAACE group F1,14=41.87, p= 0.0001 and SPG group F1,13=32.30, p= 0.0001. There was significant improvement in quality of life of participants in the TAACE group F1,14=155.79, p= 0.0001and SPG group F1,13= 29.62 p= 0.0001.Conclusion: In conclusion, Allium Cepa in as an adjunt with therapeutic exercises is effective in the management of pain, improvement of upper extremity function and quality of life among stroke survivor with hemiplegic shoulder particularly in stroke survivors.
